What the trials found
In the original studies, sexual side effects — lower desire, difficulty with erections, less ejaculate — were reported by somewhere between one and two men in a hundred. Placebo groups reported the same things at around one in a hundred, so the increase attributable to the drug was roughly a percentage point.
In most men who reported them, they settled while continuing, and in almost all the rest they resolved after stopping. That is the mainstream picture and it is genuinely reassuring as far as it goes.
The part that is not settled
There are consistent reports of symptoms persisting after stopping the drug. How often that happens, whether the drug caused it, and what the mechanism would be are all still argued about, and the honest position is that this is not resolved rather than that it is disproven.
It appears to be rare. It is also not nothing, and you are entitled to weigh a small uncertain risk against a cosmetic benefit yourself rather than have someone else decide the trade is obviously worth it.
Mood
Depressed mood and anxiety have been reported, and labelling in several countries now reflects that. If you have a history of depression, that belongs in this decision. Your PSA test, which nobody mentions
Finasteride roughly halves PSA, the blood marker used in prostate screening. It does not affect your prostate risk, but it does mean a result read without knowing you take this can look falsely reassuring.
